I'm Commander Shepard and this is my favorite rank on the citadel. Why would I want to change it?
To be fair, becoming an undead cybernetically enhanced space marine is pretty awesome already .
Who needs promotions when you can have interspecies relations in your very own cabin whenever you want?
I think they probably didn't like how Shepard smiled at them when he asked for a promotion…

To be fair, I think Commander is actually more of a position and less of a rank. I could be wrong.
But he gets plenty of achievements.

No Promotion for saving the world in mass effect. Deliver a message to the Jarl of Whiterun and get a promotion in the Legion.
I don't know if this has been posted yet but… Shawn just ruined the "new" ending of Mass Effect 3. All we'll get from the ending is Shepard gets the purple heart (or whatever they'd call it in the Mass Effect Universe probably some kooky space name) right after the last cut scene. The end. If this ends up being true I don't want to live on this planet anymore…

Shepard kinda left the Alliance in Mass Effect 2. That probably muddied things up. Though I guess it's surprising that Shepard didn't get a posthumous promotion when s/he was KIA.

1st galaxy save ended with the consel pretending that it never happened…
2nd galaxy save got him fired and hired by cerberous only to be fired at the end of the game…
3rd galaxy save ended with him dead…
