I was playing 'Don't Fear The Reaper' on Rock Band last year sometime when my friend and I also thought up the Minecraft-related lyrical change. Perhaps it's a natural source for lulz...

Is your lyrics and idea made into a poorly recorded song? I don't think so! (But in all honesty I have seen many versions of this around, I just decided to do something. And I will be rerecording this and uploaded a redux edition of the song sometime in June!)

At first I thought this was the design by Glen O'Neill: http://glenoneill.com/post/5533799495/grimcreeper

But then read in Dave Cheung's comments that an artist "previously commissioned to do it [ran] off and [stole] the idea for his own nefarious ends," so am assuming that would be O'Neill, and if so I am now okay with Dave's design being about 20 times better than the one linked above.

The Legend of Zelda: Link's Awakening

Now that I think about it, given the out-of-place characters found in that game, I don't think seeing a creeper in there would be much of a stretch.
