After you complete the 3 surgeries, you unlock the ambulance. If you somehow manage to do them all in the ambulance, you get to do the heart surgery IN SPACE.

Day 1: I write this with a sooty pebble and a scrap piece of chicken skin. It's tough out here, real tough. The other ribcages eye me up; they know I'm easy pickings. I try not to anger them. I found a space to sleep next to the dumpster and can only hope it's not been taken.

Day 2: Made friends with a small fish skeleton. It followed me back to my little corner and hasn't left my side. I think it likes me. I'm gonna call it Steve.

Day 3: I wonder how my mother's doing. Walking around without me holding up her organs. Or maybe she's dead. I look up to the stars and imagine she's watching over me like a floppy terrifying facsimile of a human.

Day 4: Steve slipped away from me today to bark at a young spine passing by. I had to run over and apologise. She tells me she's got nowhere to go either, so I did the only thing I could think of and offered up my spot.
In the light of our tiny bonfire her vertebrae shimmer faintly like stars. Now that we've got each other for company, the streets don't seem so bad. I think we'll be alright.
